Senior Services at Hurley Medical Center
At Hurley Medical Center, our goal is to keep older adults healthy, active and independent. For your well-being, or that of an older adult loved one, our team of dedicated Hurley professionals and geriatricians provide the highest level of quality health care.
Hurley Senior Services is staffed by a specially-trained team consisting of a registered nurse, medical assistant, medical secretary, medical social worker, x-ray technician, and two geriatric physicians.
To ensure that every older adult patient receives the most appropriate and timely care possible, our team uses an interdisciplinary approach to planning care, focusing not only on the physical well-being of the patient but also on the psychosocial, cognitive, emotional, family and social service needs. Our program features health promotion, disease prevention, chronic disease management, and patient and caregiver education. Our services are beneficial to a broad array of older adults, ranging from the very active to the extremely frail, including those in hospice care, assisted living, senior housing and home-bound. Patients who wish to remain under the care of their existing primary care physicians are offered a geriatric consultation, which includes a full assessment by the team, follow-up visits as needed, and recommendations reported to the primary care physician for ongoing care.
